Abnormal Regional Metabolism and Mechanical Function in Patients with Ischemic Heart Disease Improvement after Successful Regional Revascularization by Aortocoronary Bypass
نویسندگان
چکیده
Left ventricular anterior wall metabolism was investigated concurrently with global myocardial metabolism by simultaneous preoperation sampling of anterior interventricular venous (AIV) and coronary sinus (CS) as well as arterial bloods in seven patients with severe obstructive lesions of the major coronary arteries, including left anterior descending. Postoperative study was performed two weeks to six months following successful aortocoronary artery bypass surgery. All grafts including the aorto-left anterior descending artery grafts were patent. Preoperatively in three of the seven patients, anterior wall lactate extraction (R%L) was negative at rest. The average R%L at rest (7 ± 14%) was abnormal and was negative (-49 ± 26%) at a maximum supraventricular pacing rate (MPR) of 137 ± 4.6 beats/min. Postoperatively, not only was resting R%L (39 + 4.4% ) normal but also it remained normal during atrial pacing (32 ± 8.5% )

متن کاملRegional myocardial wall thickening response to nitroglycerin. A predictor of myocardial response to aortocoronary bypass surgery.
The peak rate of systolic wall thickening (p +/- dTw/dt) in regions of the left ventricle (LV) was determined preoperatively by biplane roentgen videometry in 18 patients before and after sublingual administration of nitroglycerin (NTG) and 3-23 months (median 12) after aortocoronary bypass surgery. The regional LV response to NTG was a reliable predictor for postoperative improvement in region...
متن کاملRelationship between myocardial viability and improvement in left ventricular function and heart failure symptoms after coronary artery bypass surgery.
BACKGROUND The evaluation of myocardial viability is an important preoperative parameter, predictive of improvement in regional and global left ventricular (LV) function after coronary artery bypass surgery (CABG). However, whether the presence of viability is also associated with relief of heart failure symptoms after revascularization is not always certain. The aims of the study were to defin...
متن کاملDobutamine stress echocardiography identifies hibernating myocardium and predicts recovery of left ventricular function after coronary revascularization.
BACKGROUND The identification of hibernating myocardium is important in selecting patients who will benefit from coronary revascularization. This study was performed to determine whether dobutamine stress echocardiography (DSE) could identify hibernating myocardium and predict improvement in regional systolic wall thickening after revascularization. METHODS AND RESULTS DSE was performed in 49...

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...
